Identification Cards/Birth Certificates - Cuy - Age Through 17 - Payment Assistance | Identification Crisis Collaborative
Provides assistance applying and paying for paperwork that proves identity for eligible individuals. Service may include a voucher to pay for a birth certificate, a voucher for a state ID or (possibly) a driver's license depending on individual need and situation. Documents for those born outside the US may include birth certificates and Consular Reports of Birth Overseas for US Citizens. Provides state IDs for youth needing a work permit or for applying for financial aid for post-secondary education. Cannot help with replacing a lost Permanent Resident Card (Green Card) or lost proofs of naturalized citizenship.

Application process
Walk in for service. Those without proof of Social Security number can still be referred. Agency will work with applicants to get Social Security documents that are acceptable to state agencies that issue identity and driver licenses. Vouchers are good for one month from the date issued. Those needing a state ID (or driver's license if eligible) will receive a voucher to take to the state agency that issues identification/driving licenses. Not all offices that issue Identification or driving licenses take this voucher. Organization will provide a list of sites that take the voucher. Youth applying for a birth certificate or ID in order to work must be accompanied by a parent/legal guardian.

Eligibility
Provides birth certificate vouchers for ages 4-6 who are registering for kindergarten. For ages through 17 if needed for parent or guardian's housing (public housing, housing vouchers, etc), for parent/guardians heating assistance appointment ( HEAP or PIPP) or for ages 15-17 in order to work. *Serves those whose parents are unemployed, eligible for unemployment, or receiving unemployment benefits of any amount, or who are struggling to obtain documents due to circumstances caused by the corona virus. *Youth are also eligible if their parents/guardians are eligible for food assistance (SNAP) or have income that comes only from Social Security Disability (SSD) SSI, or Social Security retirement. *Birth certificate help for those with current ID only if applicant has document stating that they require a birth certificate in addition to ID. *Applicant must not have received this service within the past 12 months. *Vouchers for State ID for those with no ID aged trhough 16 years. *Vouchers for Driver's licenses only if applicant had a license that expired no longer ago than 6 months or if they lost a license that has not yet expired. *No assistance for temporary driving permits or to get a driver license if applicant did not previously have one or if license expired more than six months ago.
