Eligibility
- Serves residents of Portage County.
- Serves adults age 65 or over or who qualify for Medicare based on disabilities and with income at 100% to 135% FPL.
- Must be a citizen or qualified legal alien.
- Assistance available will depend on income and family resources.
- Resource limits apply; $9,430 for single individual and $14,130 for couples.
