FAMILY PRESERVATION TEAM | APPLEWOOD CENTERS
Provides community support program services and skill teaching for families who have had a child placed outside the home or who are at risk. Specialized services include: intensive in-home services, reunification (for children returning home), parent-teen conflict, and assistance for caregivers in accessing community services that would improve their chances for parenting success (job searching, parenting, household management, counseling, etc.).

Referrals are through the county Dept. of Children and Family Services, but individuals may call for information about the program, M-F: 8:30am-5:00pm.

Sliding Scale if no insurance. For Cuyahoga County, Medicaid currently only insurance accepted. Accepts private insurance for residents of other counties.
Eligibility
Serves Cuyahoga County. Families who have had children removed from the home are eligible, as well as parents with teens (aged 13 and older) who are experiencing conflict that cannot be resolved without outside help, as referred by the Department of Children and Family Services.

Provides behavioral healthcare, special education, and social services for children, youth, and families. Offers a multi-disciplinary staff, including licensed counselors, social workers, teachers, nurses, psychologists and psychiatrists. Services include outpatient counseling, psychiatry, residential services, adoption, foster care, in-home and school-based services, internship programs and research partnerships. Provides the site for a residential treatment program for youth with both substance abuse and mental health issues. Operates five sites in Cuyahoga and Lorain Counties.
