Required documents
-Identification,
-Insurance information,
-Social security number of patient,
-Current medication list, and
-Custody or guardianship documentation (if applicable).
If you are uninsured,
-Proof of income, and
-Proof of residency (can be a piece of government-issued mail such as a cell phone bill, utility bill, bank statement, etc.).

Walk in assessments are available. Specific appointments can be made by calling the location. An individual must have an intake prior to receiving psychiatry services.
