LONG TERM CARE INFORMATION / OPTIONS COUNSELING | LINKING EMPLOYMENT, ABILITIES AND POTENTIAL
Provides individualized assistance to those needing help with appropriate placement planning and information on long term care services. Offers information about insurance issues, available benefit programs, and types of facilities and care alternatives, including assisted living, skilled nursing, hospice, home health, respite care, adult day services and other home and community-based care. Provides eligibility counseling and financial pre-screening for State and Federal long-term care services.
