Child and Adult Care Food Program (CACFP) enrollment and Income Form.
Year round program. School year M-F: 2:00pm-6:00pm. Summer: M-F:8:00am-5:00pm for eight weeks.
Must first enroll into the program, phone for information or to register M-F: 9:00am-5:00pm. Once registered, walk in: School year, M-F: 3:30pm-4:00pm; Summer: M-F: 12:00noon-1:00pm.
Income based. Accepts county vouchers.
