- Serves working individuals that work or live in Tuscarawas County, ages 18 to 64, who do not have medical insurance. - Household income must be at or below 199% of the Federal Poverty Level. - Veterans must contact the area Veteran's Outreach office to determine eligibility for Veteran's benefits.
Will be requested as needed and may include: - Proof of residency. - Photo ID. - Social Security card. - Proof of employment and income: Three (3) months of pay stubs, most recent tax return, proof of any public assistance received (food stamps, and so forth), if applicable. - Vision and dental insurance cards, if applicable. - If patient is a Veteran: Patient must present a letter of ineligibility for Veterans' Benefits from the Veteran's Outreach office.

Interpretive services available by appointment.

Call the office Monday to Thursday from 8:00am to 4:00pm for information and screening. All required documents must be presented at the eligibility appointment. Eligibility will be reviewed yearly. Patients are required to notify staff of any changes in their situations that would affect their eligibility to receive care from the clinic.

Provides assistance in obtaining prescriptions for individuals who are uninsured or, for other reasons, cannot afford to pay the market price. Helps qualifying individuals complete and file paperwork for submission to drug manufactors to request medication and provides financial assistance to purchase necessary prescription drugs to individuals who would be in an emergency situation without assistance.
