Required documents
Psychological evaluations and other pertinent medical records. Educational records, IEP, ETR as applicable. Referral if one was provided by another healthcare practitioner.

Provide comprehensive and focused Applied Behavior Analysis (ABA) therapy services for individuals on the autism spectrum ages 2-21. Services are available in on-site clinic, homes, schools and daycare centers. Service intensity ranges from 10 to 30 hours of one on one therapy per week.
