Application process
Phone M-F: 8:00am-5:00pm for an assessment appointment. A prescription from a physician is required for this service, but the hospital can assist callers in getting a prescription.
Fee
Third-party payment accepted, including some Medicaid plans and Medicare. Those who have no health insurance must have household income at or below 400% of federal poverty guidelines to apply for financial help. Financial Counselors (FC) will screen uninsured for Marketplace insurance, Medicaid, or hospital charity eligibility. Patients who qualify for charity might not always have a 100% of bill covered. those who have an outstanding bill may be able to receive a payment plan on a case-by-case basis.
