Proof of income (Proof of Monthly Income (2 months of paystubs or award letter from Social Security, Pension, etc.) for all household members age 18 and older),Tax preparation documents (Tax returns for the last two years for all household members age 18 and older),Other documents (Most recent 2 months of bank statements and list of monthly payments for all household members age 18 and older),Photo ID (For all household members age 18 and older)
