Call the local utility company for the medical certification form. For questions, contact the utility company or PUCO's Customer Call Center.

Eligibility

Serves residential customers of PUCO-regulated electric, natural gas, and water utilities in Ohio. Program cannot be used more than three (3) times in a rolling 12-month period and may vary based on utility company.
