Confirmation of residency. Residency proof includes current utility bill (if homeowner), lease agreement (if rental is a large complex), and if residing with someone - or if renting a single, two-family or any rental with up to five units - the owner of the property must sign an owner-tenant form verifying this, and attach proof of ownership (current water bill, real estate tax bill or home owner’s insurance policy). Parent will then provide his/her utility bill, no older than 30 days. May also need official court order granting legal custody (if divorced) and copy of IEP (for special education services).

For a child who has an enrolled sibling, must register at the school where the child will attend.
