Eligibility

Open to individuals with a disability who reside in Boone, Campbell, Kenton, or Grant County and who require assistance with certain activities of daily living. Eligibility for specific services is determined by a care manager during an in-home assessment.

Fees

Co-payment may be required. Co-payments are set on a sliding scale based on income, assets, and medical expenses.

Offers assessment and case management for individuals with disabilities to assist them with remaining in their home as long as possible. Through case management services, participants have access to a variety of in-home services, such as assistance with chores, home delivered meals, accompaniment to and from medical appointments, personal care assistance, etc.
