Application process

Phone Central Intake for an appointment M-F: 8:30am-5:00pm. Client should be able to get an assessment within one week of contact.

Fees

Sliding scale down to 30% gross income. Accepts Medicaid and Private insurance.

Description

Provides a mental health diagnostic assessment to determine the issues or problems facing the client and to aid in determining the course of treatment. Client does not have to be involved in any of the agency's programs in order to make use of this service. Cannot do custody evaluations.
