Phone for an appointment M-F: 8:00am-5:00pm. Initial screening is required.
Fee
Fixed fee. Third-party payment accepted, including Medicaid and Medicare. Those who have no health insurance must have household income at or below 400% of federal poverty guidelines to apply for financial help. Financial Counselors will screen uninsured for Marketplace insurance, Medicaid, or hospital charity eligibility. Patients who qualify for charity might not always have a 100% of bill covered. Those who have an outstanding bill may be able to receive a payment plan on a case-by-case basis. For patients with insurance: program does not cover out of pocket expenses such as co-pays, co-insurance, and/or deductibles.

Ages 18 and over, diagnosed with diabetes.
