Provides a housing program designed to increase independent living skills and self-sufficiency through a non-clinical peer support model. Goal is permanent housing. Program includes support services, assistance with establishing educational goals if wanted, skills to maintain behavioral health, manage finances, and develop social supports in the community.

Individuals' case managers may contact the county board to request a referral. Board will contact the organization to check on availability and contact eligible applicants to provide the application process. May also call organization M-F: 8:30am-3:30pm to check availability.
Fee
If applicants have income or benefits, they should expect to pay a monthly per diem rate.
Eligibility
Ages 18-25 who reside in Cuyahoga County who are recovering from mental health or substance use disorders and are in need of permanent housing. Must have a case manager with a mental health agency.
