Diabetes Information/Management | Cleveland Clinic Foundation - Hillcrest Hospital
Provides a comprehensive diabetes education program that includes group and one-on-one teaching of all phases of diabetes management, including the physiology of diabetes, insulin instruction, meal-planning, hypoglycemia management, and blood glucose monitoring. Also provides periodic community education about the disease.

Community education classes are offered 4 times a year. New diabetes management classes start monthly.Individual consultation is also available M-F: 9:00am-5:00pm.

Application process
Call one to two weeks before scheduled program to register for the program. For diabetes management or for individual consultation, call and leave a message to register 24 hrs a day, 7 days a week. To stay informed about changes to this schedule, may also call to be put on mailing list for regular calendar updates. Staff will call back.
Fee
Fixed fee for diabetes management. Third party payment accepted, including Medicare and Medicaid. Also offers basic, medically necessary hospital-level services free of charge to individuals who are residents of Ohio, and who are currently eligible recipients of state low-income or disability programs, or whose income is below the Federal Poverty Income Guidelines. Provides financial assistance on a sliding scale to patients who do not have insurance at family income levels up to four (4) times the Federal Poverty Guidelines, and to all patients, including patients with insurance coverage, if there are exceptional circumstances.

Ages 18 and over. No residency restrictions.
