- Serves women who are pregnant, breastfeeding, or have had a baby (or been pregnant) within the last six (6) months.
- Women younger than 18 do NOT need parent/guardian permission.
- Serves infants and children up to five (5) years old.
- Applicants must live in Carroll County, meet household income guidelines, and meet certain nutritional or health risks.
- Income eligibility guidelines are up to 185% of the Federal Poverty Guidelines that were in effect the previous April 1st. Number in household includes unborn children (Example: A pregnant, single woman is a household of two). Definition of income does NOT include household assets.
- Individuals receiving Medicaid, Ohio Works First or Food Stamps automatically meet income eligibility.
-ID that shows name and birth date for those in the household who are applying for benefits (for the baby, will accept a crib card from the hospital).
- May need proof of pregnancy from doctor or clinic, unless the pregnancy is clearly visible.
- For income verification, will accept the following: three (3) pay stubs, SSI statements, Medicaid card, proof of enrollment in Ohio Works First, Food Assistance Card, a written statement from child's father saying he is paying child support.
- Will also need proof of the child's current immunizations and proof of residency (current utility, telephone, or credit card statement).
- Those who are not the mother, but who have custody of the child (such as fathers and kinship caregivers) will need proof of custody. This may vary
