Must be ineligible for PASSPORT, in need of one or more Care Coordination services, and in need of a service not available through other community services. Special priority is given to applicants who have the highest level of impairment, have the lowest income, are members of a racial minority group, live alone, or have no caregiver.

Description

Provides help to older adults who want to remain independent and living in the community. Enrollees receive personalized services that are coordinated by a care manager. The care manager can assist with things like disease management, health insurance benefits, service coordination and transitional care. Services may include adult day care, chore, homemaker, home delivered meals, personal care, safety monitoring and minor home repair.

Plans, funds, and coordinates services for older persons and persons with disabilities in Portage, Stark, Summit, and Wayne counties to live with dignity and choices in their homes and communities for as long as possible. Administers PASSPORT, MyCare Ohio, The Assisted Living Waiver, The Ohio Home Care Waiver, Care Coordination, Elder Rights, Family Caregiver Support Program, Nursing Home Placement Assistance Program, and other services designed to help older adults and individuals with functional limitations maintain independence. Provides funds for services administered by other community agencies including client finding, specialized information and referral, transportation, home-delivered meals, medical treatment, personal care, respite care, safety monitoring, health assessments, congregate meals, recreational activities, and adult day care.
