Provides mental health services in the home and in community settings in order to enhance level of functioning, develop coping skills, reduce the need for more restrictive services, and encourage good personal choices. Services include assessments, personal interventions, and family involvement as well as development of community living skills, educational, and vocational functions. Provides telephone and in-person support during crisis situations, as well as case management to help the individual access appropriate community resources.
