Children under the age of 23 who have special health care needs and are residents of the State of Ohio.
Specific Program Eligibility:
*Treatment Program: both medical and financial eligibility are required. Services may be renewed each year until the child reaches age 23.
*Service Coordination Program: limited program available to children, with specific conditions, who are seen by a team of providers at hospitals approved by CMH for service coordination.
Mon-Fri 8am-4pm
CMH-approved doctors must send medical application form to CMH. Call or email for more information or for a referral to a CMH-approved doctor.
