Serves Ohio counties: Allen, Auglaize, Darke, Mercer, Miami, Paulding, Preble, Shelby and Van Wert and Indiana counties of Jay, Randolph and Wayne. Serves those experiencing a life-limiting illness with a life expectancy of 6 months or less; those who no longer wish to seek curative treatment for their illness; those who wish to remain in their home with a focus on comfort and symptom management; those in a facility setting who would benefit from extra support during their life-limiting illness.

Description

Provides hospice care that focuses on the individual's confort and goals during their life-limiting illness. Program offers everything from music therapy for pain and symptom management, to assistance from social workers and chaplains. Services may include professional nursing to assist with medical needs, aides to assist with personal hygiene and activities of daily living; licensed social workers to provide assistance with future planning and to ensure the safety and comfort of each patient, chaplains available to provide non-denominational spiritual and emotional support, board-certified music therapists to assist with pain and symptom management, and also offers emotional support, volunteers to provide assistance with light housework, quick errands, companionship and caregiver respite.
