Eligibility
Serves Medicaid recipients in Summit County. Appointments must be medically related and within 50 miles. Appointments further than 50 miles require additional approval.
Required documents
Information Needed:
- Participant's name, address (including zip code), phone number, Social Security number or Medicaid case number.
- Appointment date and time.
- Name of medical provider/facility, appointment address (including zip code, and building, hospital, or suite number, if applicable), and phone number.
