Eligibility
Serves Medicaid recipients in Summit County. Appointments must be medically related and within 50 miles. Appointments further than 50 miles require additional approval.
Required documents
Information Needed:
  • Participant's name, address (including zip code), phone number, Social Security number or Medicaid case number.
  • Appointment date and time.
  • Name of medical provider/facility, appointment address (including zip code, and building, hospital, or suite number, if applicable), and phone number.

By appointment. All trips must be scheduled at least five (5) business days before the medical appointment.
