Provides initial screening that may lead to arranging an appointment for an in-home Coronavirus (COVID-19) vaccination, including booster shots. Vaccination is provided by outside organizations such as the local fire department, National Guard, etc. The vaccine provided (Pfizer, Moderna, Johnson & Johnson) will depend on availability and client preference. Client preference may affect how long it will take to receive the vaccine (for example, if the client wants the Moderna vaccine, and none is available at the time, the client may have a longer wait for service).

Application process

Phone anytime to set up initial screening. In most cases, client will need to leave a voice mail and will be called back by this agency within five to seven business days for an initial screening. After the screening, the client's contact information will be passed to a vaccine provider who will contact the client to arrange service. If making a referral for someone else, be prepared to provide their name, date of birth, phone number, and address.

Eligibility

Serves homebound persons in Cuyahoga, Geauga, Lake, Lorain, and Medina County. Homebound persons include those who need the help of another person or medical equipment such as crutches, a walker, or a wheelchair to leave their home, or their medical provider believes that their health or illness could get worse if they leave their home, therefore they typically do not leave their home.
