Valid photo ID or passport, if patient has neither, then 2 alternate forms of identification such as birth certificate, social security card, hospital discharge summary, etc.

We accept Medicaid, Medicare, and most forms of insurance. If your insurance won’t cover your treatment for some reason, we will work with you to get you the help you want.
