Provides meals to plan participants who have been recently discharged from the hospital and who have conditions such as food insecurity and key, chronic medical conditions, including congestive heart failure (CHF), cancer, chronic obstructive pulmonary disease (COPD), diabetes, HIV/AIDS, and end stage renal disease (ESRD), and pregnant and postpartum members with gestational diabetes.

Application process

Call or go online for information and assistance with getting service. During the COVID pandemic, Medicaid enrollees were not required to reverify to make sure they were still eligible for Medicaid benefits. The hold on these reverifications has ended and reverification reminder notices will be rolled out throughout the year. Enrollees should look out for reverification packets in the mail and report any address changes to their JFS office.
