Application process
Phone for an appointment M-F: 8:30am-4:30pm. Service is one on one. For adults, accepts referrals from the Cuyahoga County Board of Developmental Disabilities (CCBDD), Opportunities for Ohioans with Disabilities (OOD), and other agencies who work with the eligible population. For children, accepts referrals from the school district, physician or self referral.
Fee
Fixed fee. Third party payment accepted, including Medicaid and Medicare.
Eligibility
Ages 16 and older. No residency restrictions. Clients must be able to arrange their own transportation. Must be independent in their personal care or medication needs, or must provide their own attendant. Must be able to get around in a wheelchair on their own. Qualifying disabilities include epilepsy or cerebral palsy, brain injuries, muscular dystrophy, spina bifida, multiple sclerosis, or spinal cord injuries.

Advances the independence, productivity and full citizenship of people with cerebral palsy, spina bifida, epilepsy, muscular dystrophy, traumatic brain injuries, spinal cord injuries, autism spectrum, genetic disorders, Down Syndrome and other developmental disabilities. Offers early intervention and family support services, traditional physical/occupational/speech/language therapy services for children, intensive physical/occupational therapy services, residential services (offers several group homes), an assistive technology center, and organizational and community employment services.
