Eligibility
Residents of Ohio at or below 200% of the Federal Poverty Level, without dental insurance and who do not qualify for Medicaid (unless they have spend-downs). Clients must have a reliable source of monthly income. Also sees those living in Ohio without legal permission who therefore do not qualify for Medicaid. Will only see children under 18 if they are in need of braces or live in Ohio without legal permission.

Completed application (available online). Must include complete financial information and proof of income.

Telephone. Website. After receipt of application, a referral coordinator will contact client and discuss eligibility.

Free or sliding fee scale, based on eligibility.
Service area
Franklin County, Ohio
Links clients with dentists or clinics where they can receive dental care for reduced fees. Dental care includes dentures, partials, root canals, wisdom teeth extraction and braces for children and teens. Does not provide preventative care, emergency care, cosmetic care (veneers, whitening), bridges, adult braces or implants. Clients may be placed on a waiting list, depending on the available number of volunteer dentists in their area.
