Eligibility
Open to any family in Kentucky who has a child diagnosed with at least one of the following health conditions: Asthma/Cystic Fibrosis, Autism Spectrum Disorder, Cerebral Palsy, Cleft Lip and Palate, Congenital Heart Defects, Craniofacial Abnormalities, Diabetes, Epilepsy, an Eye Condition, Hearing Loss/Ear Disease, a Muscle/Ligament/Joint Condition, Rheumatoid Arthritis, Scoliosis, or Spina Bifida.
