Outpatient Addiction Treatment | Eagle Healthworks

1. Eagle Healthworks accepts patient who call or drop-in to the clinic. Valid photo ID (state, driver's license, prison, military)and proof of insurance required. If self-pay call for rates. 2. After initial intake, we will perform a biopsychosocial assessment to determine appropriateness of treatment. 3. Next, you will provide a urine drug screen and alcohol breathalyzer test to determine current substance use and intoxication. 4. Next, you will either see the provider for a full evaluation and diagnosis, or be scheduled.
