Provide air conditioners to individuals 60 years and older or those who has a medical disability with a doctors statement during the summer. Last 30 days of Income for all household members 18 years of age or older, Self-employed required 12 months of income (must have form filled out and IRS 1040). You must obtain the self-employment form at your local HEAP office, prior to your appointment, Seasonal Employees must provide pay stubs, a printout or seasonal employment verification form for the past 12 months-you can obtain this at your local HEAP office, prior to your appointment, Zero Income must have documented proof of how you met your personal needs including rent/mortgage, food, utilities, medical and any assistance received and if someone has given you money for rent, bills, etc., a letter from the gift giver with the amounts and their name, address, and phone number Proof of citizenship for all household members (ex. social security cards, birth certificate) Proof of medical condition (documentation signed and stamped by a licensed physician or registered nurse practitioner) \, Proof of disability (if applicable), Renters: Lease or statement from landlord or metropolitan or HUD housing worksheet Winter crisis program runs October - March.

Summer Crisis all appointments are in person interviews only. Runs July 1, - Sept 30. To schedule an appointment call, 513-970-6737 or online at https:// wccs.itfrontdesk.com . Warren County residents can visit www.energyhelp.ohio.gov to start their application prior to their required agency interview with WCCS.
