INDIVIDUAL SUPPORT COORDINATION | FAIRFIELD COUNTY BOARD OF DEVELOPMENTAL DISABILITIES
Individual Support Coordinators (ISCs) support clients through coordination of services including social, medical, education, and employment. Services may include: *Identify the person’s strengths and resources. *Determine the needs of the person and work to find resources to support those needs *Create a plan that helps individual reach their goals *Connect individual and family with resources and help coordinate between multiple agencies when needed *Check in regularly to discuss updates, changes, and successes *Family supports are also available.

Ages 3-5: *At least two areas of developmental delay and a qualifying diagnosis as documented by a qualified professional. Ages 6+: *Diagnosed developmental disability from a qualified professional *Eligible according to the Children’s/ Ohio Eligibility Determination Instrument (C/OEDI).
