Medical History Information forms in office.
Hours
Mon-Thu 8am-5:30pm Fri 8am-12pm Closed weekends and holidays.

Call, message through Facebook or website, or email for appointment.
Fees
Many private insurances and all medicaid accepted. Sliding fees for services are based upon a client's income and family size, however, no one is denied services due to their inability to pay.
