Eligibility
Anyone residing in Licking, Knox, Perry, or Fairfield County with payer source such as Medicare, Medicaid Waiver, passport, VA, BWC, insurance or ability to pay privately. Must have physicians order which can be obtained by by Interim prior to client admission.
Required documents
Provide SS # or Medicaid/Medicare #, any insurance policy #'s, DOB, address, Physician's name and contact person name.

Provide a higher level of personal care, shaving, bathing, skin care, catheter care, colostomy care, turning and positioning, assistance with ambulation, transfers to wheelchair, etc. Nurse aides can do vital signs and remind patient to take medication as set up by RN. Available minimum of 2 hours up to 24 hours a day by 8 hour shifts. Supervised every 2 weeks by RN. RN on call 24 hours a day. Plan of treatment submitted to physician. Also offer companion and homemaker service.
