Eligibility
Serves youth and adult residents of Lorain County and surrounding communities.
Required documents
- If available, a valid photo ID and medical or insurance card. Medicare, Medicaid or insurance cards need to be presented at every visit so that health care coverage may be verified prior to being seen by the provider.
- If applying for financial assistance, a Sliding Fee Scale (SFS) application form must be completed at the Health Center and submitted for approval along with proof of household income. Proof of income includes prior year's tax return, at least two (2) pay stubs from current employer, unemployment receipt, disability determination, Social Security statement, retirement benefits statement, and any other income. Verification of SFS status must be updated annually and as needed.

Fees
- Accepts Medicaid/Medicaid Managed Care, Medicare/Medicare Managed Care, many commercial insurance plans, and private pay.
- Minimum fee of $20 for medical clients who qualify for a 100% discount based on Federal Poverty Guidelines.
- Sliding scale based on income,/household size and other financial assistance available.
