Application process

Phone for information and assistance with applying, or complete the Family Application Form online and provide verification of income. Go to www.odh.ohio.gov, and click on letter H in the A-Z index, scroll down to Hearing Aid Assistance Program. Licensed Audiologists and Hearing Aid Dealers and Fitters must have a signed agreement in place with the department before providing service. Once forms and documents are completed, applicants return the forms to their hearing aid dealer.

Fee

Sliding scale to cover audiological services to assess the child and follow up to evaluate aid.

Eligibility

Serves hearing impaired Ohio residents 21 years of age and younger from families with incomes at or below 400% of federal poverty guidelines. Children who qualify for Medicaid or the state's program for children with medical handicaps are not eligible for this program.
