Direct Patient Assistance Programs | Kidney Foundation Of Ohio
Provides assistance in meeting the costs of medicine and emergency needs when no other means are available. There is a limit of $400/calendar year ($200/semi yearly) for assistance with medication grants. There is a one-time/year amount of up to $100 for emergency grants. Emergency needs may include, but are not limited to, utilities and car repair. Offers transportation assistance in the form of mileage reimbursement, bus passes, and cab vouchers (on an emergency basis) for dialysis and transplant patients. Makes available personalized medical alert tags. No assistance is offered for rent, mortgage, or lease payments. Also offers financial assistance to those with kidney disease who are pursuing higher education.

Application process
Phone for information M-F: 9:00-4:30pm. An application must be completed for most services. It is preferred that a social worker work with the client on the application and that the social worker submit the application. There may be a waiting list for some services. Organization will mail an application for higher education assistance to eligible individuals who call to request it.
Fee
Free. For financial aid with higher education, payment is either made directly to vendors or reimbursement to those who qualify.
Eligibility
Serves people who are receiving dialysis or are pre-dialysis or who have received a kidney transplant. There is a limit for transportation assistance. In Ohio, serves individuals who live in or dialyze in the following counties: Cuyahoga, Erie, Huron, Richland, Lorain, Ashland, Medina, Wayne, Holmes, Lake, Summit, Stark, Tuscarawas, Guernsey, Noble, Morgan, Athens, Meigs, Gallia, Lawrence, Washington, Monroe, Belmont, Jefferson, Harrison, Carroll, Columbiana, Portage, Geauga, Ashtabula, Trumbull, and Mahoning Counties. In West Virginia, serves Brooke, Ritchie, Wetzel, Wirt, and Wood counties.
