Medicaid - Long Term Care (FORMERLY NURSING HOME DEPARTMENT) | Portage County Department Of Job And Family Services
Provides public insurance to cover medical services to those of any age who require long term care, which may include doctor visits, some prescription drugs, hospital care, home care, long-term care, medical equipment, transportation, mental health, vision, dental, substance abuse services and other services. Medicaid is the payer of last resort - Medicare will be billed first if client is eligible for it.

Application process
- Call the office to schedule an appointment if desired. - Initial application may also be submitted through website, fax, email or by mail (once application is received, client will be notified of additional documents needed or appointment time if requested). - Clients who may be eligible for Medicare will be referred for enrollment.

- Serves residents of Portage County and be a United States citizen - Individuals of any age who need long term care. - Financial eligibility is an income at 300% Federal Poverty Guideline Level and assets up to $2,000 for an individual, $3,000 for a couple, those whose income is higher must create a Qualified Income Trust (QIT).
