Offers one-on-one assistance to help individuals determine which private, public, state and local benefits they may be eligible to receive. Includes benefits such as Medicaid, Medicare Part-D, SSI, SSD, SNAP (food stamps), HEAP, Golden Buckeye Card, Homestead Exemption, prescription assistance programs, housing and others. Will assist clients with filing out and submitting applications and forms and provides follow-up with client to see if they were able to enroll in the programs. Uses the Benefits Check-up screening tool.
