Eligibility
U.S. Citizen 18 or over who have not had an eye exam in 12 or more months and are determined to be at increased risk for glaucoma based on age, race, and family history of glaucoma. Cannot belong to an HMO or receive eye care benefits through Veteran's Affairs.
Required documents
Referral letter needs to be presented to doctor's office.

Application process
To determine eligibility visit website for online referral questionnaire. If eligible, will receive a letter in the mail in 1-2 weeks with the doctor's information.
Fees
Uninsured clients are seen at no charge. Insured clients are responsible for co-pays.
