Provides for the safety of elderly individuals by giving them a red magnetic pouch to keep on their refrigerator containing vital information needed which would alert paramedics, ambulance attendants and other emergency personnel.

Application process

If receiving other CoA services, the social worker will complete the Vial of Life during the assessment and/or home visit. If not receiving other CoA services, you may call and request that a Vial of Life be mailed to you. Option to have information typed and kept in a database at the CoA. Routine reminders to update the Vial of Life are mailed.

Fee

Service is free but donations are accepted.

Serves Lake County residents age 60 and older.
