- Serves residents of Portage County.
- Clients may visit once a month.
Required documents
Valid photo ID (State ID or Driver's License). If current address is not on the ID, will need to bring a recent piece of mail as proof of having a Portage County address.
Hours
Must call to schedule an appointment. At this time, appointments will be scheduled for:
- Monday: 1:00pm to 3:00pm.
- Wednesday: 1:00pm to 3:00pm.
- Friday: 10:00am to Noon and 1:00pm to 3:00pm.
