Must meet all of the following criteria: - Must meet income guidelines. - Home must be owner occupied. - Must have current homeowners insurance. - Must be current on property taxes. - Must have a citation from the Health Department.
Completed application with all sections of the application that applies to the applicant completed. Must submit applicable information from the required documentation checklist. Permits applicants to make copies of documents required with submission of the application at the office. Documentation check list: - Copy of homeowner’s insurance listing dwelling (house) dollar amount of coverage and annual premium. - Copy of most recent W-2 Statement of Earnings or #1099 Statement. - Copy of most recent Federal Tax Return #1040 (must be signed and dated). - Copy of three (3) most recent pay stubs. - Name, address, phone number, and dates of employment with all employer(s) for the past two (2) years. - Copy of Social Security Benefits Statements (Form SSA-1099, SSI, or SSDI). - Copy of pension stating monthly or yearly earnings. - Copy of complete divorce documents/decree, if applicable. - Copy of bankruptcy discharge (only if occurring before the three (3) year limit). - Copies of the last three (3) months bank checking/savings account statements. - Copy of any health district orders.
