Eligibility

Ages 18 and older. For pregnant women, offers outpatient treatment but they must be under the care of an OB/GYN or they will be sent to one. To receive free care must be resident of Cuyahoga County and must have income no higher than 100% of the Federal Poverty Level (FPL).

Required documents

Health insurance card valid photo I.D if no insurance, must have proof of county residency: accepts a utility bill or bank statement no older than 30 days.

Outpatient treatment: M,Tu,Th,F: 9:00am-12:00noon M-Tu-Th 6:00 - 9:00pm A pretreatment group for indigent heroin addicts is held W: 1:00pm (MUST be on time to be admitted).

Application process

May walk in for assessment M-F: 7:00am-9:00am. May also phone M-F: 8:30am-7:30pm for information and to arrange for assessment. May leave a message after hours that will be returned, usually within the next day.

Fees

Third-party payment accepted (some Medicaid, Medicare, some Private insurance plans); if uninsured, will attempt to get patient enrolled in various available city, state, or national insurance programs for which they are eligible.. May qualify for patient payment assistance, which is a discounted bill based on income. Those with no insurance must meet income limits to qualify for free care.
