Coordinated Intake | Frontline Service

Conducts a coordinated intake assessment to help homeless individuals and families find or maintain a safe place to stay. * If assessment results indicate that families/individuals are eligible, will place those who have been assessed in an available shelter. The goal is to help the client establish both a short- and long-term housing plan. *Due to shelter space limitations and client circumstances, there is no guarantee of immediate placement. * There is no on site day care.

Application process

On the day shelter is needed, eligible individuals/families/couples may call for assessment. Assessment is required before receiving ongoing shelter placement or any secondary services. Those who are not literally homeless on the day they call will not get service or a returned call from the agency. After pressing 1, callers are presented with a range of additional extensions, including Ext. 1=ages 18-24; Ext 2=veterans; Ext 3=all others. Leaving a message at the wrong extension can significantly increase the amount of time it takes to get a call back. *FAMILIES: Those who reach voicemail at any time can leave a message with their names, phone numbers/emails. Calls will be returned usually within 30 minutes. Those leaving a message should listen carefully to the prompts to ensure they reach the program they want to be considered for. *Families without access to a phone M-F: 8:00am-6:00pm should go to the Zelma George Family Shelter at 1710 Prospect Ave., 44115. They should tell ZG staff they are homeless and without a phone and that Coordinated Intake told them to go to ZG to use the phone for intake screening for shelter. *SINGLES: Single adults who reach voicemail during regular hours can leave a message with their names, phone numbers/emails. Calls will be returned as soon as possible, usually within 30 minutes. *M-F: 6:00pm-8:00am, holidays and weekends, single adults should NOT leave a voicemail message. They should go to either Norma Herr (women) or 2100 Lakeside (men) to stay and should call back during regular hours. *Single adults without access to a phone number should go to either Norma Herr (women) or 2100 Lakeside (men) at any time and should call Coordinated Intake for an assessment during open hours. All callers will be fully assessed by phone for shelter placement. Intake assessment outcome determines if applicants will be placed in shelter.

Eligibility

* Serves single adults (male or female) age 18 or older, veterans, and families who are physically present in Cuyahoga County. Serves couples age 60 and older. Both members of a couple must be age 60 or older. Serves those who are in a substance disorder rehab facility in Cuyahoga County if they were literally homeless at the time they entered rehab. A family is at least one adult who, at the time of the call to 211, has a minor child with them that they have custody of. * Must be literally homeless at time of call to Coordinated Intake. Literally homeless means they are living outside, living in a condemned building, living in a car, or living in a dangerous environment. * A dangerous environment includes those who are being stalked or human trafficked. Those whose dangerous environment is domestic violence must first contact the domestic violence hotline for assessment. * Those being referred to shelter must be willing to go to a homeless shelter. * A Pregnant woman is considered a single individual. *Eligible individuals or families with a member who is positive for the virus COVID 19 will need to be isolated from others in shelter. They should tell staff their status at the time of intake. * Living in a home that is infested with pests (bedbugs, cockroaches, etc) or in a home that does not have utilities connected (water, gas, electricity) does not qualify as homeless when seeking shelter. * Accepts those with dogs or mini horses who are service animals. Accepts those with emotional support animals who are with them at time of call on a case by case basis. *Shelters are accessible to those who use a wheelchair. However, part of the assessment for shelter includes being scored on the Activities of Daily Living (ADL) assessment checklist. If an individual cannot: administer their own medication, feed themselves, bathe/toilet independently, or get up off the floor independently, they are not appropriate for shelter placement. * Does not accept pets.
