Application process

Cards are automatically mailed to residents 60 years old with valid state ID. Those without a state ID, or with a disability may apply at most libraries or senior centers.

Residents of Ohio 60 years and older, or age18-59 permanently/totally disabled as defined by Social Security Administration.
