Bring Medical Card if available and photo ID. For proof of income to be considered for sliding fee schedule, one of the following documents will be accepted: 1.) One month of pay stubs 2.) a tax return or W-2/1099 from the most recent tax year 3.) Social Security benefit verification letter.

Description

Substance Use Disorder services include one-one-one counseling with a drug addiction counselor; administration of Vivitrol is available from a healthcare provider: Naloxone doses available to prevent overdose deaths. Counseling provides individuals with an opportunity to actively pursue emotional healing and personal growth, in a safe and non-judgmental environment. Although our difficulties are unique, we all face trials of many kinds. Facing these trials, with the support of a counselor, can be an empowering experience. Recovery Support Group SMART Recovery is FREE and is held Tuesday evening. Support group help people learn self-empowering tools and members will provide support for each other in recovery. The goal of SMART Recovery is for you to achieve a healthy, positive and balanced lifestyle so you can conquer new challenges. Groups are held at the Knox County Community Health Center - Mount Vernon, Ohio location (Health Department) in the Conference Room on Tuesday evening at 6 pm. This group meets for about an hour and is open to anyone, no matter where they are at in their journey to recovery. There is no need to pre-register, and there is no fee to attend the support groups.

The Community Health Center is a patient-centered medical home offering preventive and primary care to for men, women and children of all ages, including those who have no insurance, are under-insured or have insurance.
