Lake County household fuel customers with total household income at or below 175% of poverty guidelines. Both homeowners and renters are eligible. (175% of Federal Poverty Guideline)
DOCUMENTATION TO BRING TO THE APPOINTMENT: -Photo ID (driver's license) -Social Security cards for every member of the household -Income verification for the past thirty (30) days for every member of the household. (If self-employed, if individual had no income or insufficient income then they will need a tax transcript (obtainable from the IRS) from the most recent tax year. Self employment, odd jobs and seasonal jobs must provide most recent tax forms, all pages included Schedule C or tax transcript. Must show they were submitted to IRS if self prepared. - All school employees must provide 12 month income or contract showing they are paid 12 months of year. -Gifts: Money the individual does NOT pay back. A statement written and signed by the person who provides the money. Must include dates and amount within last 30 days. Must include name, address, and phone number of person. -Utility Reimbursement: Section 8 verification. -VA Benefits: Current award letter that states benefits are service or non-service related. -Loans: Money you DO have to pay back. A statement signed by the person who provides the money. Must include dates and amounts within last 30 days. Must include name, address, and phone number of person. Must provide dates of repayment. -Current statement of child support (current printout from DJFS site or payment card dated within 30 days worth of history) If you pay child support and can provide documentation, it will be deducted from your income calculation. -Unemployment: printout of award letter or pay stubs detailing gross amount and deductions -Social Security: Award letter for current year or bank statement -Pension: Current statement showing gross amount and deductions. PERS must show statement. -Cash assistance (ADC/OWF): Printout with 30 day history showing gross amount and household demographics -Verification of health insurance premiums individuals pay monthly. -If renting, landlord information (name, address, phone number, and monthly rent amount). If bill is in landlords name agency will verify name on account. Landlord must provide applicant with statement that he wants to keep bill in his name but client is responsible to pay the bill. A copy of the bill must also be presented. -Current electric and gas bills (all pages) NOTE: If utility is off, applicant must have a Pending Account Number from the utility company. NOTE: Painesville City Utilities customers must have an authorization release on file at the Painesville City Utilities Office. -If applicable, proof of any required co payment.
