Application process

Clients may begin the application process, renew, and/or update their case information by calling the Contact Center Monday through Friday from 8:00am to 4:00pm. - May also apply, renew, and/or update case information through website 24 hours a day, 7 days a week. NOTE: Client must have an email address to do this. In most cases, any interviews can be done through the Contact Center. - If applying by phone, the interview may be done during the phone call. - If applying by website or paper, the agency will schedule an over the phone interview. Client will be given a day and time frame to call in. Agency can look up most supporting documents through an electronic records search. The client will be notified if they need to supply any extra supporting documents. - If preferred, may also pick up and drop off paper applications and submit documents on-site Monday through Friday from 8:00am to 4:30pm.

Eligibility

- Serves low income residents of Portage County households with at least one (1) child under the age of 19. - In a household with one adult, the adult is required to work or be involved in specifically defined work activities at least 30 hours per week. In a household with two (2) adults, those adults must meet a 35 hour per week requirement. Cash assistance is time limited to three (3) years. After a 2-year waiting period, families in need of additional assistance may reapply for another two (2) years. - Cash benefits are capped at 5-year lifetime limit.
