HNCO provides comfort, support, and care for families dealing with the last phase of life. Our care team gives guidance and assurance and journeys with patients and families. Physical, spiritual, and emotional support is provided in the patient's home or where the patient lives. Palliative care is also available for those dealing with chronic illnesses, even if they are seeking curative measures. Free bereavement services, including individual counseling, support groups, and workshops, are provided for the families of hospice patients, and also to anyone in the community living with grief.

Application process

For hospice and palliative care, a physician's referral is required. HNCO can obtain that for the family. Please call to arrange. Bereavement services are available without referral.

Fee

Hospice care is covered by Medicare, Medicaid, private insurance, or self-pay methods. No one is turned away for an inability to pay. Palliative care is also covered by most insurance, and visits are charged like a doctor's visit with a co-pay. Bereavement services are free.

Eligibility

Hospice patients typically have life expectancy of six months or less. Patients with advanced, life-limiting chronic illness are eligible for palliative care. Anyone living with grief is eligible for bereavement services.
